Medication Renewal Policy

To help us renew your prescription on time, please remember:

• ​Review your medication needs at each appointment.
• Bring your medications to each appointment with your Doctor or Nurse Practitioner.
• Check the amount of your medications between appointments to make sure you have enough.

If you run out of medications before your next appointment:

• Call your pharmacy and have them fax your renewal to us at:

  Gananoque Site Fax: 613-382-7323
• Allow a minimum of ten calendar days for us to respond to your pharmacy.
• Please do not call the LGCFHT for renewal requests.
