Community Diabetes Education

Individuals living with diabetes, or pre-diabetes, learn to manage their condition, enabling them to live healthier lives.

Participants learn how to test their blood sugars, recognize, and manage, low and high blood sugars, develop healthy meal plans, learn about ways to cope with stress, and how to exercise to control blood sugars, as well as management and prevention of diabetes related complications.

​Appointments are provided during the day – and cover a variety of topics essential to diabetes management. Our services, available at the Diabetes Education Centre, include counselling, regular follow-ups, insulin starts, and follow-ups, gestational diabetes, pre-diabetes and those at risk for developing diabetes.

​Whether you have pre-diabetes, type one, type two, or gestational diabetes, the diabetes team will collaborate, with your health care provider, to ensure optimal health.

​The diabetes team accepts self-referrals and referrals from physicians and other health and social service providers (including telemedicine, and endocrinology).
