To inquire about enrolment with one of our providers, please call Health Care Connect.

Health Care Connect assists people in finding a Family Doctor or Nurse Practitioner.

A resident of Ontario must have a valid Ontario health number. This number can be found on your Ontario health card.

Health Care Connect will need your health number to confirm that you are eligible to participate in the Health Care Connect program.
